Permission to Attend Junior Camps

As per EMHA Directives, any Player trying out for a Junior Team must have a ‘Permission to Try Out’ form signed by EMHA. This form can be downloaded below, and the Player must contact the EMHA Registrar, Nadine Shimizu, to have it signed. If permission is not obtained the Player is deemed an ineligible player and consequences are indefinite suspension.

GRADUATING MIDGET PLAYERS:
Graduating Midget Players must obtain permission up to April 30, 2010 (the end of the current hockey season). After April 30th Graduating Midget Players do not need to obtain permission.

EMHA's preference is that your local Registrar (Tracy de Zwaan) with any questions you may have regarding filling out the form, and once it is filled out please scan and email it to her at zone8registrar "AT" shaw.ca and CC Tracy at registrar "AT kchockey.caso that we know who’s going where. If you do not have access to scan and email, it can be faxed to 780-463-5736, but you must call that number first to have her switch the fax machine on (it’s on the same line).
